The inaugural Aflac All–American High School Baseball Game was played on August 23, 2003 at Hammond Stadium in Ft. Myers, Florida where 38 of the nation's top high school baseball stars faced–off in the first annual East vs. West matchup. Established as a fundraising event for pediatric cancer, proceeds from the Game are donated to the local children's hospital in the host city; in 2003 the Game's beneficiary was the Children's Hospital of Southwest Florida. The 2003 Game capped off a week of events including an awards banquet the night before the Game and the Home Run Derby finals that took place a few hours prior to the first pitch. Shortstop Christ Nelson took home the Game's first MVP award as the East defeated the West 5–3.
Hall of Famer and Former St. Louis Cardinal's Outfielder and First Baseman Stan "The Man" Musial served as the Aflac All–American High School Baseball Game's first Honorary Chairman. The Game's first class was immensely successful in the following MLB June draft, producing four of the top 10 picks of the 2004 draft in Matt Bush (1st overall), Mark Rogers (5th overall), Homer Bailey (7th overall) and Chris Nelson (9th overall).
In 2004 and 2005, the Aflac All–American High School Baseball Game found a new home at Cal Ripken Jr. Stadium in Aberdeen, MD, with future Hall of Famer Cal Ripken, Jr. serving as the Honorary Chairman in both years. The 2004 Game introduced the Jackie Robinson Award – an honor bestowed upon the Aflac National High School Player of the Year who demonstrates outstanding character, exhibits leadership, and embodies the value of being a student–athlete. Rachel Robinson presented the inaugural award to future No. 1 draft pick Justin Upton. The 2004 Game also produced four of the top 20 picks in the 2005 June draft, including Arizona Diamondbacks standout Upton.
The 2006 and 2007 Aflac All–American High School Baseball Games were played at San Diego State University's Tony Gwynn Stadium and saw Reggie Jackson and Ozzie Smith serve as Honorary Chairmen in each year, respectively. In the 2007 Draft, 13 2006 All–Americans were selected in the first round of the June MLB Draft, the most ever for the Game. In this year's June MLB Draft, 10 former Aflac All-Americans were picked in the first round. Tim Beckham, MVP of the 2007 Aflac Game, became the third Aflac All-American to be selected first overall in the MLB Draft.
Continuing its philanthropic mission, the Aflac All–American High School Baseball Game donated proceeds in both 2006 and 2007 to Rady Children's Hospital and Health Center in San Diego, as well as to Coaches Curing Kids' Cancer, a program funded by the Curing Kids' Cancer Inc. – an organization devoted to raising money to support cutting edge cancer treatment for children. To date, the Game has raised $525,000 for pediatric cancer.
